About the role
Role Overview
Backend Engineer focused on high-scale, high-throughput platform engineering for a multi-tenant healthcare technology platform. The system processes tens of thousands of background jobs daily and is built on an event-driven architecture.
Responsibilities
- Build and maintain high-performance backend services using Node.js and TypeScript in a modular monolith (Express + GraphQL)
- Design and optimize large-scale background job processing (throughput, retries, idempotency, backpressure, fault tolerance)
- Orchestrate asynchronous workflows with Google Cloud Pub/Sub, Cloud Functions, and scheduled jobs (App Engine Cron)
- Improve performance via profiling, PostgreSQL query optimization (indexing/query planning), and Redis caching
- Design resilient distributed systems to handle retries, partial failures, outages, and high traffic
- Build and maintain integrations with third-party services (payments, messaging, healthcare APIs) reliably at scale
- Collaborate on infrastructure and CI/CD pipelines using GitHub Actions and GCP services (App Engine, Cloud Storage, BigQuery, Secret Manager)
- Implement observability and monitoring (Cloud Logging, Winston, alerting dashboards) for fast incident response
- Contribute to secure multi-tenant architecture (authentication, RBAC, data isolation)
- Occasionally support full-stack features when needed (Vue 3 + Apollo GraphQL)
Requirements
- 5+ years of backend or full-stack engineering experience in production
- Proven experience owning high-throughput job-heavy backend systems (queues/workers/background processing)
- Strong expertise in Node.js and TypeScript, with solid API and system design skills
- Deep experience with distributed systems and asynchronous/background job architecture
- Strong PostgreSQL performance tuning (indexing, query planning) and ORM usage (Prisma preferred)
- Experience with queues/messaging systems (Pub/Sub; also RabbitMQ/SQS/Kafka or similar)
- Familiarity with caching/performance optimization (Redis)
- Ability to design for reliability, idempotency, and fault tolerance under load
- Cloud infrastructure experience (GCP preferred; AWS/Azure transferable)
- Strong debugging and incident response mindset
- Good communication and comfort working in a remote, async team with high ownership
Nice to Have
- Pub/Sub, BigQuery, or Cloud Functions experience
- Background in healthcare/fintech or other regulated high-reliability domains
- Experience designing multi-tenant SaaS systems
- Familiarity with rate limiting, backpressure, and system degradation strategies
- Exposure to AI/LLM integrations or data-heavy pipelines
