Back End Developer

Role Overview

You will be an early backend hire responsible for PeterMD’s server-side systems, including the API, data model, integrations, and financial correctness. The platform must accurately model state over time for revenue recognition, cohort retention, and LTV, while also supporting new operational surfaces.

Responsibilities

• Own the backend API: build and maintain Vercel serverless Node.js + TypeScript handlers with authenticated role gates (patient/provider/admin/super-admin scopes).
• Design and maintain the Postgres data model on Supabase: additive migrations, Row Level Security (RLS) policies, audit triggers, PostgREST behavior, indexing/query performance.
• Handle authentication and scheduled backend work via Supabase Edge Functions.
• Model financial state over time:
  • transaction lifecycle events
  • revenue recognition events
  • subscription plan and shipment sequencing
  • cohort anchors
  • include historical backfill
• Handle payments correctly:
  • Stripe migration from a legacy processor (phased rollout; verified batches)
  • webhooks, tokenization, and deferred revenue-aware billing
• Integrate pharmacy fulfillment across six partners/APIs (routing, document generation, dry-run defaults, live sends only when requested).
• Ingest lab results via S3 delivery and HL7 result matching (Ulta Lab Tests).
• Support legacy commerce during migration:
  • WooCommerce order webhooks and subscription handlers
  • Shopify + Supliful supplement fulfillment (live during migration)
• Connect communications, scheduling, and CRM:
  • Twilio SMS
  • AWS SES transactional email
  • Zoom appointment provisioning
  • Meta Conversions API
  • bidirectional sync with GoHighLevel
• Support the AI layer: service/evaluation layer behind the in-product clinical assistant using the Vercel AI SDK.
• Maintain shared domain packages (pure logic, tested first) with Vitest.
• Ship safely despite limited safety nets: feature flags, allowlist canaries, and versioned releases with generated checks (and no existing CI; merging to master currently deploys to production).

Requirements

• Strong backend engineering experience delivering correctness in production systems.
• Proficiency building serverless APIs in Node.js/TypeScript.
• Experience with Postgres, data modeling, and safe schema evolution.
• Familiarity with Supabase concepts such as RLS, PostgREST, migrations, and Edge Functions.
• Experience integrating third-party systems via webhooks/APIs (payments, fulfillment, lab data, CRM).

Nice-to-haves (from the posting)

• Experience with financial/event-driven domain modeling (temporal models for revenue recognition/cohort metrics).
• Stripe migration or payments domain experience (deferred revenue, tokenization, webhook handling).
• HL7 and/or lab results ingestion experience.
• Testing experience using Vitest.
• Experience working in environments with evolving CI/CD practices and implementing release safety mechanisms.
